I'm in another pickle; I can't remember my admin password. What is the
best way to recover/reset it?

    I believe the ControlledNameTypes table went away a couple
    of years ago and doesn't appear anywhere in 1.4.0 except for
    possibly the upgrade code that's run during installation, so
    my guess would be that you've got some old PHP code hanging
    around in an HTML file that you customized, that makes use of
    that table.
