[SPT/CWIS] Keep getting "This page cannot be found" when adding a record.

When using SPT, using the Metadata Tool to add a record, when I put in 
the information and click to actually add the record, I get "This page 
cannot be found", the record is actually added however.

Another database, running on the same installation of SPT, has no such 
problem, although the two databases have different look-and-feel 
configurations.

Has anyone experienced this problem?  Any ideas?
